Services Through Partnership Affiliates Remain Open as of March 25

Dear Friends and Community Partners,

We know that many services available for our mutual clients and patients have temporarily been reduced or may not meet the needs of vulnerable populations who cannot adequately access remote/ telehealth services. The Partnership for Successful Living affiliates – Keystone Hall, Harbor Homes and Harbor Care Health and Wellness Center, Healthy at Home, and Southern NH HIV/AIDS Task Force – remain here to help. Of course, this is subject to change in response to best practices around COVID-19, but as of March 25,  all of our services remain open. With new and appropriate protocols to keep our providers and clients safe, we offer both in-person call-ahead services whenever feasible, as well as remote, telehealth services.

Services include:

  • Primary and behavioral health care, as well as pharmacy services and Medication Assisted Treatment, at our Harbor Care Health and Wellness Center clinic, a Federally Qualified Health Center. Both in-person and telehealth services are available. Call 821-7788 for more information.
  • 24/7 behavioral health crisis services in Greater Nashua through our Mobile Crisis Response Team. Call 816-0101 for more information.
  • Residential substance use disorder treatment for men, women, or pregnant/ parenting women and their children (in-person) and intensive outpatient and outpatient substance use disorder treatment (telehealth options) through Keystone Hall. Call 881-4848 for more information.
  • Both in-person and telehealth home care services, including personal care service, RN services, LNA services, and more, through our in-home health care agency, Healthy at Home. Call 595-4243 for more information.
  • Telehealth HIV/AIDS services, including case management and supportive services, assistance with benefit enrollment, and access to short and long term housing assistance through the Southern NH HIV/AIDS Task Force. Call 595-8464 for more information.
  • Access to housing for individuals and families who are homeless or at risk of homelessness, including permanent supportive housing for special populations, such as those who are chronically homeless and veterans. Call 882-3616 for more information.
